For females: CrCl = [(140 – age) × weight] / (72 × serum creatinine) × 0.85

BSA-adjusted CrCl = CrCl × (1.73 / BSA)
BSA is calculated using the Du Bois formula: BSA = 0.007184 × height0.725 × weight0.425
