Revised Trauma Score (RTS)
RTS = (0.9368 × GCS code) + (0.7326 × SBP code) + (0.2908 × RR code). RTS ≤4 indicates need for trauma center.

GCS code: 4 for GCS 13-15, 3 for 9-12, 2 for 6-8, 1 for 4-5, 0 for 3. SBP code: 4 for >89, 3 for 76-89, 2 for 50-75, 1 for 1-49, 0 for 0. RR code: 4 for 10-29, 3 for >29, 2 for 6-9, 1 for 1-5, 0 for 0.
