ADA Standard Formula: eAG (mg/dL) = (28.7 × A1C) – 46.7
To convert A1C to mmol/L: eAG (mmol/L) = ((28.7 × A1C) – 46.7) ÷ 18. For glucose to A1C: A1C = (eAG + 46.7) ÷ 28.7
